Japan faces the world's most acute dementia challenge: 7 million people currently living with the condition, projected to reach 10 million by 2030 as the most aged major population on Earth grows older still. For most of those people, the current care pathway ends in a locked memory care ward — an environment that neurologists increasingly recognise as actively harmful to a brain that retains emotional memory long after factual memory has gone.

The Dementia Community Living Initiative, funded at 800 billion yen over ten years in the 2023 Care Infrastructure Act, is constructing 200 purpose-built dementia villages across Japan by 2030, with 47 already operational. Each village houses 150 to 200 residents in domestic-scale houses grouped around shared spaces — a supermarket, a hair salon, a garden, a cafe — that replicate normal neighbourhood life. Residents move freely within the secure perimeter. Staff work in plain clothes. The environment is designed around the premise that familiar, meaningful activity is itself therapeutic.

Outcome data from the first 20 operational villages, published by the National Center for Geriatrics and Gerontology in 2024, shows antipsychotic medication use among residents is 71 percent lower than in matched conventional memory care facilities. Documented agitation incidents are 58 percent lower. Family satisfaction scores are 40 percent higher. Staff turnover — a chronic problem in conventional dementia care — is 35 percent lower, attributed to the greater meaningfulness of relational rather than custodial work.

The model has attracted particular interest for its cultural adaptation: each village is designed around a specific generational lifestyle reference — one in Kyoto replicates a 1960s market town; one in Osaka centres on a neighbourhood bathhouse and a covered shopping arcade.

"Dementia does not erase the person," said program director Dr. Naoko Yamamoto. "It erases the recent past. We build for the past they still have."
