Sickle cell disease affects approximately 2 percent of births in Ghana — one of the highest prevalence rates in the world — and has historically been managed rather than cured: pain crises treated with opioids and hydration, organ damage slowed with hydroxyurea, survival extended through blood transfusions that carry their own risks. The gene therapies approved in the United States and United Kingdom in 2023 offered something categorically different: a single treatment with a high probability of functional cure. Their list price, at the time of approval, was $2.2 million per patient.

Ghana's path to offering this treatment through its national health insurance system required three simultaneous negotiations: with Vertex Pharmaceuticals and bluebird bio on a differential pricing arrangement tied to per-capita GDP, with the Global Fund and Wellcome Trust on grant financing covering 60 percent of treatment costs for the first five years, and with the Korle Bu haematology unit on the infrastructure upgrades required to perform lentiviral vector gene editing at the required safety standard.

The resulting programme offers gene therapy to eligible Ghanaian sickle cell patients — those under 35 with severe disease and a matched donor available for the concurrent stem cell harvest — at a patient cost capped at GHS 4,500 (approximately $290), covered by the National Health Insurance Authority for registered members. One hundred patients have received treatment in the programme's first 18 months; 94 of them have achieved haemoglobin F levels consistent with functional cure.

The model is being studied by Nigeria, Cameroon, and the Democratic Republic of Congo, all of which carry significant sickle cell burdens and are in early-stage negotiations with the same manufacturer consortium.

"We were told this was not possible in Africa," said Professor Kwaku Ohene-Frempong, the programme's clinical director. "We chose not to believe that."
